If the veteran is deceased, a son or daughter is considered next of kin and can request a copy of the veteran's DD-214 (Report of Separation) from the National Archives. If the veteran is living, the veteran generally must request the DD-214 themselves or provide written authorization for someone else to obtain it.

If requesting the DD-214 for a deceased parent, the requester should be prepared to provide:

• The veteran's full name used during military service 
• Branch of service 
• Dates of service (approximate is acceptable) 
• Date and place of birth, if known 
• Service number or Social Security number, if available 
• Proof that the veteran is deceased (such as a death certificate, obituary, or funeral notice if requested) 
• Proof that the requester is the veteran's son or daughter, if needed
